It means the person who made the movie is letting you know ahead of time that what you're watching may have never taken place, but the movie industry is going to try and sell as being true anyhow.
Take for instance the Texas Chainsaw Massacre. I saw the remake with Jessica Biel and I was horrified. The reason I saw it was because I had noticed the ad said it was based on a true story. However the ';true story'; it was based on was about a man named Ed Gein who liked to wear people's skin. He killed two people. He never had any help from friends or family, and never even used a chainsaw, but hey, according to Hollywood, it's close enough to use that type of advertisement for the film.

I agree with the 2nd chick who posted; it's so they can pass it off as near-fact. It probably didn't happen at all like in the movie a lot of the time.
What happens is that Hollywood is an industry that revolves around money, and the people creating films want to cash in as much as possible on a good story (or what they think is a good story). So they can hear a story but elements of it might not be 'commerically viable' or detract from what they see as a movie that could net them a lot of money. So they'll change parts, leave out parts, create people, create roles for actors who they may want to cast in the movie, etc.
Sucks but it's true with many so-called 'based on true events' movies.
Some details are added...Take for instance, Hoffa, the Danny Devito character never existed, he is just a character created based on many different people in Hoffas life.
haha i think they mean they're using the plot of what really happened, but changing around some of the details
